How to complete Intermittent FMLA Paperwork, HR is no help?
I am trying to secure intermittent FMLA to care for my mother who has been diagnosed with cancer. She doesn’t drive, and while I try to get all of her apointments before I begin my shift (1pm-10pm), it’s not always possible. I had the paperwork filled out by her surgeon, and HR said it was filled incorrectly. The main question was does thie patients condition qualify under the categories of:
1-Hospital Care
2-Absence Plus Treatment
3-Pregnancy
4-Chronic Condition
5-Permanent Long Term
6-Multiple Treatments
The doc chose #2, but this is not the right answer, and HR says they can’t tell me what the right answer is…she has been in the hospital for two surgeries, has to have multiple treatments, and I suppose this could be a long term or chronic condition. Stage III Breast Cancer
Any thought on what should be chosen as the ‘right’ answer?
Maybe chronic condition? I don’t like their limited categories. I wonder if they match the language in FMLA – it seems like there is more. I would find out more about FMLA. There’s probably a lot of info on the web on it. The medical secretary should have experience with this at your doctor’s office – better yet if it’s an HMO because they are much bigger and the medical secretaries KNOW which ones get approved by both the doctor and the employer.
I find this odd though because I am on intermitten FMLA – up to 6 days a month for Dr.’s appointments and personal care. Our doctor approved it and for patient privacy, my employer is not privy to the reason. Although my employer gave me a form, my HMO gives them THEIR form which my employer accepted. Maybe I was lucky but I did use the services of the HMO’s medical secretary.
Good luck – it’s hard when you have to put your job on the line over the health of a loved one.
